Referral Fee Information

HIPIOWA will pay a $200 fee to Iowa state licensed insurance agents who assist applicants in filling out an approved HIPIOWA application form.

Information required of the agent to process the commission payment:

1. Photocopy of current insurance license.
2. Completed IRS Form W-9. Click here to download a W-9 form.
3. Completed Section VIII of the insured's application.
